THE FINAL DAY OF THE PROGRAM PENDEDAHAN KERJAYA SEORANG DOKTOR AND WE SAW THE BIRTH OF A NEW BORN!

Woah, its the 5th day already and I don't want to go back!I want to stay on as a DOCTOR!LOL.But,reality has sunk in and the fact is finishing my pre-u is my first priority now.Anyways, today was the day i appreciated all the pain my mum went through to bring us into this world. The day I sa a mum in labour.Thank you for letting us watched madam. We arrived early and our group was supposed to go to the neurological department but instead we asked for permission to go to the O AND G.When we arrived at the labour room,Sister Norliza separated us into 3 groups each consists of 5 people. Then, when my group entered the labour room, the loady was already in labour pain. But first, let me explain to you the normal process of giving birth:

a)The vagina should be at least 10 cm for the baby to come out.The vagina will increase in length every 1 hour 1 cm.
b)I f the increase in length is slow,a medication called Ritosin is to stimulate pain in the vagina. If the patient is very sensitive, she will give birth in no time.
c)Now,if this does not occur,They will have to use a suction pump to bring the baby out.
d)And if this fails,a Caeserean operation will have to take place. They will make an incision below the umbilicus(belly button) to take the baby out.
e)And yeah, the perineum, the region between the anus and the vagina must be guraded to ensure it does not tear to cause excessive bleeding.

Now,back to our case, the mother was in labour pain.We were really nervous and scared. There was a machine taht was connected to the mother abdomen to detect the heart rate of the baby and had a graph reading being printed.I was really speechless when the mother was asked to push several times and still the baby could not come out. They waited for a few minutes and tried again.Still cant come out.The MO arrived and was really stern.Almost like shouting at the mother to push harder but the best part was when the specialist came in.He was super cool, took control of everything and tried to use the suction pump to take the baby out.He was 'tegas'but had good manners with his patient. But sadly, this too couldnt bring the baby out,so they brought her to the operation theater to do Caserean.Wow,too bad we couldnt see the baby, but we got to see a lot of processed before it could be delivered.

After that,we were just lepaking at the guest area while filling up our forms.Well, lets a fast forward a little. Now,we are at the neurology wards. There was not much explaination by the sister in charge so here are 2 of the cases we saw.The first patient had a tumor and a swollen head. The 2nd patient had an accident and needs fisiotherapi to assist him recovery. This was at the male wards. When we went to the female wards, that was when a cool doctor who studied in glasgow, scotland gave us so many advice on the life of a doctor.Well, what is job a doctor.Basically they have to follow these few steps called diffential diagnostics:

a)History of the patient
b)Physical examination
c)Resustitation(this is suppose to be the first step during an emergency case)
d)Find the cause
e)Treat by using medicine or surgery

Owh yeah,as an housemen, make sure everything is done before going to your senior(either medical officer or the pakar) during the ward rounds and you must know each a every patient's problem during the rounds. For examplae,if someone with appendicitis the first pain is below the belly button but later part it might be more on the right. So,this is an important thing to know before making a diagnostic.
And another thing is that during a emergency, always call the MO OR THE SPECIALIST if you cant handle it yourself.ALWAYS ASK FOR HELP as everyone works in a team.Speak openly with others, dont be shy and dont be rude.Very important thing on becoming a doctor.THE MOST IMPORTANT THING IS TREAT YOUR PATIENTS WITH RESPECT NO MATTER WHAT.Owh,and she gave us a few tips for the interview with JPA.Read up on the current issues and voice must be loud and clear.Really good advice on the last day.God sent this angel to talk to us.Thank you very much DR MIZATUL LAILI.

After lunch,we headed back to the Pusat Bersalin for the final taklimat. They put on a video of HSAJB tihe their motto,KAMI MEMBANTU ANDA SIHAT.Short and sweet.LOL.And then, there was a presentation by the Deputy Pengarah Hospital. There were slides on the road of becoming a medical doctor. Here are few of the important things:

a)A levels/pre U
b)Entry to a medical school
c)Housemanship
d)Provisional registration with MMC
e)Acquisition of apointment with SPA
d)Letter of placement by MOH
e)Houseman training for 2 years
f)Under the supervision of medical officers and pakars
g)Must fill up log books

And another thing is doctors should follow these principals in life:ABCDE CONCEPT

a)Appreciation
b)Be positive
c)Commitment
d)Discipline
e)Eveready and energetic

And here are few quotes that inspire me to become a doctor:

a)Satisfaction does not come with achievement, but with effort.Full effort is full victory
b)Everything should be made a simple as possible but not simpler
c)If you don't ask,you don't get.

AND LASTLY JOIN THE HANDWRITING CLUB.HAHA.Well,this is the end of the program but a new beginnig for me and everyone else who wants to be a doctor.I will be a doctor and I will fulfill my medical mission.I wish everyone else good luck in achieving their dreams and I AM gonna miss everyone especially group 1.Hope my readers enjoying my interpretation of the events.Take care and appreciate life!

4TH DAY OF PROGRAM PENDEDAHAN KERJAYA SEORANG DOKTOR AND WE WENT TO MONASH UNIVERSITY!WOOHOO!

4th day already.Time really flies when your having fun.And again,we got another posting.Urology department.Something to do with the kidneys, bladder, ureter and urethra.Anyways,we headed to the urology department and met with a sister there.I forgot her name .Haix, having bad memories these days.Must be the on call I guess.HAHAH.the department was really small, only 25 beds and only 12 patients, mostly post surgery cases.Mr Kalidasan was the head but we didnt get to see him.Anway,we got to interview some patients:

a)1st patient was a man who was admitted for the first time from 12/2009.He had some kind of cancer and inflammation of the kidney. He has a restricted diet and has to be admitted every three months to undergo a procedure.
b)2nd patient was also a man,who had water in his lungs, underwent a surgery to remove one of his kidney which the wound was now infected.Poor guy, imagine the pain and suffering. He has tubes connected to his chest to remove excess water or liquid.
c)The next patient was a teacher.Wow,imagine knowing a teacher who has health problems.SAD SAD.He had a cancerous growth on his bladder but only certain part of it could be removed. He has to come to the hospital occationally to do 'irragation' of the bladder which means(clean out all the blood)Btw,the irrigation liquid used is 3000ml saline that can last to 2 to 3 hours.

Next stop was the orthopedics ward. All about bones, bones and bones. We just randomly went there and there is one particular patient we were interested in.There were two policemen guarding him and we asked the ma few questions.HAHA.The best part was they thought we were doctors and they quickly scrambled away their chairs and stuffs.LOL.Anyway,he was explaining about the patient who was a convict that injured himself while trying to escape.Broke his arm. They had to 'jaga' him in hospital until he is found guilty in court. And yeah,an advice to all doctors, treat anyone no matter what race or religion or background he is from.We must even treat convicts. So thats the job of a doctor. And yeah,as usual,we were out of departments to visit and started to wonder around.AND GUESS WHAT,WE WENT TO THE FISIOTHERAPI DEPARTMENT!haha,it was not even in the schedule man and yeah,we were the only team to visit that place.GO GROUP 1!Fisio was really fun too!The lady explaining to us was very nice. She showed us around(without knowing that we were not suppose to be here)haha.Equipement such as electroterapy for severe injuries such as knees and back pains. And yeah, what is the difference between a hot and cold pack.Well,a cold pack is used to treat acute pain,to reduce inflammation at the wound and help blood clotting during a recent injury.A hot pack however is used to relive pain after 2 to 3 days. The infrared ray machine is used to treat back pain and small steps are used for patients to relive their walking abilities.Wow,and then we went tot he gymnasium.HAHA.really nice to watch people recovering from their injuries. There was a wrist movement manchine, ultrasound for back sores, shoulder wheel and wax therapy to treat hand and finger injuries. The weight suspension is used to work out the muscles to their normal condition.Wow,it was really all worth it.We left the jabatan feeling a sense of satisfaction. We saw what other groups didn't and it was a cool experience!

Unfortunately, our loitering antiques came to a grinding halt as we went to the jabatan Perubatan Nuklear. The place was full of people and we messed with the wrong staff. The head lady was stern and demanded where our sister was.Wow,luckily I told Charwinth to act blur and tell sorry for tersalah masuk to this jabatan to Puan Salamah.Thank God we escaped punishment!Whew, that was the last time we ever went to an unscheduled department.BUt the story would certainly be a shocker!Seriously,we were dead bored and didnt know what to do.We don't have anywhere to go!Gosh,Su li came up with a wild idea to go and hav a visit to Monash.Woah,if the hospital finds out,we are dead.but again, whats life without risks. So,we headed off to the university!HAHA,scary but true.It was out of bound and high above a hill, but we managed to get there! And this was the day that I was soo damn determined to study there! Thanks Su li for the visiting idea! The campus was simply superb. As we entered a student from Botswana introed us to Kak Faizah, the receptionist over there. She was so friendly and referred us to a sister whose name i forgot, but she was awesome! She took us in and showed us around the campus. She was so happy(I guess they rarely get visitors)The campus has been operatinal for 4 years now and they are building a new one which is much bigger nearer to the Sultan Abu Bakar Mosque. And yeah.the old building will be completely turned inot a library.Cool.And yeah.good info for me,the 5th year rotation will be done for 6 months in Australia!OMG.The country is awesome. We entered the lecture room and its was wnoderful.Fully furnished and simply a condusive environment to produce good doctors.Had a quick photo session before going downstairs for a drink. The staff there were so welcoming and they were really nice people.I LOVE MONASH UNIVERSITY!

OMG,after that,we still managed to go to the O&G department to watch a baby being delivered.Wow,sadly,no one was gving at the moment but the sister was super kind to show us around.AND YEAH,SHE CALLED US ALL DOCTOR.It gave me feeling good sensation when she called us that.Felt really good.HAHA.Yeah,i'm Dr Suman Pillai btw.LOL.This building is quite new and it was flly airconditioned. There was a bilik saringan which is the checking and waiting time before a mum goes into labour. And the Deawn Bedah, where the Caeserean operation is done.ICU labour room is for mum's with disorders during pregnancy such as high blood pressure, diebetes adn fits. And another thing is the drug(Tocolysis)a medicine for the mother to tahan sakit before delivery. On the whole, there is about 15 dewan bedah and they have a shortage of staffs.HAHA.Another warning is HUSBANDS ARE NOT ALLOWED INTO THE LABOUR ROOM UNTIL THE WIFE GIVES BIRTH.Any idea why?Because there was once a case that the husband slapped the doctor, thinking that the doc was harming his wife.WTH DUDE.HE'S WAS JUST HELPING MAN!Hmm,anyways, the sister told us to grow up to be doktor yang prihatin dan berkaliber! And also manage time well, study hard and save sometime to take care of our own health after becoming a doctor.Owh yeah, before i forget,we made an appointment with her to see a delivery tommorrow morning!Cool!I cant wait. And yeah, she explained the process of child birth but I 'LL EXPLAIN IT tommorrow to get a better view after seeing the real thing!Really curious about tommorow, but thankful to GOD for giving me a wonderful experience today!Tommorow and tommorow and tommorow.LOL.Peace.

3RD DAY OF PROGRAM PENDEDAHAN KERJAYA SEORANG DOKTOR AND WE SAW DEAD BODIES!

Three days into the program and time is flying quite slow compared to my days in Renaissance College.Well, the slower the better because thnigs are getting very interesting!Today,we were posted at the Jabatan Pembedahan.HAHA.Don't think that we got to witness a live surgery because we're not allowed.Dissapointed but at least we went to the wards. But first,we headed to the Klinik Pembedahan where they conducted minor surgeries.A friendly officer took us on a tour.He basically told us that the Ketua Department was Mister Andrew Gunn, andowh yeah, when someone specializes in surgery,he's no more called a Doctor,he's called a Mister or if she's a lady,Madam.Nice.But we didnt get to see him though.SAD SAD.After that,we headed to the operation room but only got to look from outside because a surgery was being conducted! And yeah, there are different surgical units such as Urologi,Plastic surgery and Orthopedics. When we went to the wards, our sister explained that there were main post surgery wards(Urology is separated by male and female)and the paed ward was for children from 1 month old to 12 years of age.

Before exploring the surgical ward,2 housemen had sometime to talk to us.The guy was super jovial and very chubby.HAHA.The female doctor was explaining a lot on life being a housemen.(Sorry, but I forgot their names.LOL.)Anyways,their advice was
a)Be polite to everyone.
b)Be humble eventhough you know something they don't(They refers to the consultants and nurses).
c)A housemen is posted almost 4 months in each dept. There are 6 main departments.
d)Time management is very important.
e)Respect everyone especially nurses because they help you a lot.
f)1 log book for every 3 months posting, fill it up with everything u've learnt. Then, the consultant will mark it.If He is not satisfied,u'll be extended for a longer period.
g)Once you've become MO'S,we can take specialization courses such as MRCP from the UK.
h)Medicine is a life long learning process. It does not stop at the degree level only.

Well, after a good advice from them,we were allowed to interact with few of the patients. Here are some of the cases there:
a)Young boy met with a motorbike accident.Cracked his skull and is hospitalized for 2 months already.Aged about 2 years old.Quite serious condition.God Bless Him.Hope he'll recover well.
b)Another young boy, aged 2 years old suffered from burns as hot water fell on his leg.Wow,that hurts.3 days in hospital.(I got kena that before when I was twelve.
c)The next case was a 5 year old just after a urinary tract operation. He's 12 days in hospital.
d)And the final case was a boy about 1 years old has plastic surgery on his legs because of some burns. They change the dressing everday to keep the wound free of germs.

Anyways, after lunch was the most interesting part.We went to the Forensics Department, that is commonly known as 'RUMAH MAYAT'.Wow,it sends chills down the spine. We kinda like ponteng surgical ward because we've covered everthing. So,we headed of to the rumah mayat with another group.Very interesting place but creepy.Mr Ampri was there to guide us.First he explained that the pakar only came if the case was a murder. He had his own post mortem room taht all the cool equipments. If it was mayat Islam, the mortuary only kept it for 3 days.If it is unclaimed, the government will sponsor it to be buried. For non-muslim bodies, they keep it for one week.If it is not claimed, they body maybe be taken in for medical study purposes. We went around and saw the place where they stored specimens for analysis and the freezer for dead babies.Thy crazy thing was before we were about to leave,a nurse said excuse me to me and placed a dead baby/foetus body right beside me.Super fresh!Wow,really didnt expect it.It was all covered with white cloth.Creepy.After that,it was even more crazy. We saw 4 dead bodies in the 'peti sejuk'.The 1st body was a burnt victim, unknown identity, for more than a year already. It was fully covered with fungus and had the skull and all.Wow,but not so smelly. The next body was an Indonesian maid who killed herself by jumping off a building. She had a huge sharp crack in her skull, like someone just axed it,and not so smelly either. But,when the guy opened the third one.the smell was terrible! It was like super super super salted fish!Wow,there were two unknown bodies, the white cloth covered in blood and liquid.Had no dea how they died, but super smelly.Disgusting but thats the life of a doctor! After that,we headed to the room with all the equipments. There was a saw to cut skulls and a vacuum to suck all the bone dust. The room had a metal bed where the disection would be done.An yeah,I forgot to mention that we also saw a fresh dead guy who just died from the ward.Wow,what a day man!And yeah, before we left I saw a poster that said:

Before entering the room:
a)Have good faith.
b)The body has got nothing got to do with you.
c)Confidentiality.
d)Do not take photos or record videos.

Surely, when I become a doctor.I will follow all the rules and regulations before entering the morgue. The day was fantastic and exciting. This is a superb experience that i'll never forget. But,theres more to come and I can't wait for tommorow!Till then, wait for my update!

2ND DAY OF PROGRAM PENDEDAHAN KERJAYA SEORANG DOKTOR AND IT WAS ALL ABOUT BABIES!

Wow,exciting and exhausted but still forced myself to blog. Its the 2nd day,we were posted at the paediatrics department! The day started as usual except for the part that they didn't provide us with breakfast.Well,no a big deal anyway.Group 1 including me headed wtih Sister Zaleha to the paediatrics clinic at the main polyclinic.Upon arrival,we were introduced to the Doctors before heading to watch our first follow up check up.A specialist and her housemen were conducting them. The 1st patient was a small Malay girl with the urinary tract infection. The cool part was the doctor approached the girl in a very friendly manner and it was like the girl was not on a visit to a doctor, more like a niece visiting an aunt. The 2nd patient was an Indian girl with a bacterial infection and was on antibiotics and another Chinese boy was having backaches. He was laughing all the way when the doctor was examining him,he felt the tingles i guess.LOL.And lastly, the fourth room was very interesting.A housemen was retriving a blood sample from a small girl, and we were suprised that it was totally a different procedure from the adults. The only thing I can explain is that they don't use a syringe at all.WEIRD BUT TRUE.Sorry, but I cant really explain the procedure.Anyways, the housemen had a very good advice for us,D IS FOR DEDICATION AND THAT IS ALL IS NEEDED IN MEDICINE.

After that,we headed to the paediatric wards at block A and block B.Owh yeah, before i forget,Sister Zaleha was explaining to me on how the cannister system works.Well, basically you just dial the number of the department you want the specimens to be sent(such as blood sample or urine sample)and place the cannister inside the machine that kinda like 'teleport' the stuff out. Then, wait for a moment at the cannister just goes out from sight.Simply superb. They started thsi system in 2004.Then,we just visited some patients in the childrens wards, and met a couple of Monash University students who were doing their rounds. They were kinda busy, but still managed to spend some time with us,explaining life in med school.About 12.30pm we headed to the cafeteria to have lunch and the best part was we really blended in like Doctors.HAHA.nice.

After lunch,we straight headed to the paed wards again to have a look at the semi ICU wards where all the new born babies were. Some of them were alright while some were premature and had some complications. The 1st section was for premature babies, and another a isolated ward for babies with penyakit berjangkit such as chicken pox.We looked around and saw so many babies. Some were healthy and ready to go back home while some had certain cases such as genetic diseases involving enzyme production. There was even a baby with severe jaundice and her skin was all yellow in colour!Premature babies about 800 grams and above can still survive in HSAJB but in foreign countries, babies at 400 grams may even survive with advance medical equipments. For a baby to leave hospital safely without any other complications, they had to be more that 1.8 kilograms. And yeah, the price of an incubator for one night is almost RM 1000.00 in private hospitals, but in the government its free.Great! And yeah, before i forgot,we 'lepaked' at the doctor's room for a while and found an MRCP past year papers book. Its a specialization course for doctors and the questions are all about diagnostics and treatment method.Wow,at least I even got to see a past year medical course paper. So cool.(Rather than past year SPM papers)LOL.Well, thats all for the 2nd day,not too interesting but really a knowledgeble one.I will keep everyone posted for the 3rd day!Signing off,Dr Suman Pillai.LOL.(Feeling like one already)!

1ST DAY OF PROGRAM PENDEDAHAN KERJAYA SEORANG DOKTOR AND WE WERE 'ON CALL'!

I promised myself to update everyone on this programme and at last i've found sometime.Wow,being a doctor IS NOT EASY.Anyways,my day started quite well.I arrived about 8 am,and there we're already a number of people there. And guess what, not even one guy was wearing 'formal attire'(the complete neck tie and shirt outfit)LOL.Luckily when I approached the registration area,I,Charwinth and Kumar were the pelajar contoh.LOL.Now,lets fast forward a little bit.Met a few guys,Thivagar and Foo Siek Min,who were friendly. The hall was filled up in no time by future doctors.

Then,we were invited to eat,but I only drank coffee.Didnt feel like eating. At about 9.30am,we headed to the dewan for a taklimat. The Penolong Pengarah was giving a speech on the seriousness of becoming a doctor. It deals with lives of the patients,so yeah.Exactly about 10am,we were separated into 9 groups and I was in Group 1.Charwinth was the Ketua btw,all the girl voted for him.LOL.Really interesting. And another suprised came together,WE WERE ON CALL FROM 5PM TO 10 PM.That was super cool!Everyone was excited and were looking forward to emergency and accident cases.

At last, our group headed to our first ever posted department JABATAN PERUBATAN.Owh,yea, before I forget,my group mmebers were Foo Siek Min from Kulai,Me,Charwinth,Goh Su Li,Shaktis and Shobana from SSI JB,and a few other from all over JOHOR whose name i cant remember. Now,we followed a Sister(the term used for Ketua Jururawat)to the normal Medical Deparnment. The policlinics were packed with patients and I realised the workload the Doctor's had.The hospital was lacking in doctors btw. After that, the real fun part came.We met 2 MO'S(Medical Officers).Their pangkat is slightly higher than the HO'S(Housemans)Dr Ismail and Dr Wong gladly shared their experience about life being a Doctor. The main thing is you are not going to have an easy life as work is the main thing in becoming doctor, and dont be a doctor if your parents told you too.

Then,we headed to the wards, the reality of becoming a doctor. We were further separated into 2 groups and me,charwinth, shobana,su li and foo were on the same ward. The first patient was a ex-policeman named Abu Bakar. He was suffering from lung infection and was on antibiotics for a week.Antibiotics are used to fight disease causing bacteria in case you didnt know that. The second patient we met was admitted because he lacked blood.Another suffered from asthma and high blood pressure. The last patient we met was suffering from lymphoma(not sure of the spelling). Its the cancer of the lymph nodes. That was really terrible to hear but that a doctor's job.A doctor should never get emotionally involved with a patient. The best part was he had a huge smile and was only 42.Poor guy,I hope he gets better.

Wow,we were exhausted already and we still had our on call from 5PM TO 10PM.Imagine the doctors who work 36 hours(they come to work on monday morning and go back on tuesday evening but this is what that really excites me,the seriousness of the career. At 5 something,we headed to the A&E department(Accident and Emergency). This is where the real stuff happens. The department is separated into 3 zones. Our Sister, explained that the red zone is for the extremely serious cases, yellow zone is semi-serious cases, the green zone is for the not serious cases. Before reporting to that department, our group took a detour on our own,it was really fun.We went to the O&G department(Obstetrics and Gynecology). This place was super cool.Sister's showed us around the 1st class wards and finally we got to see a baby being delivered! It was awesome, except for the part that the girls did not allow us to watch it.Damn.Anyway, i will witness it one day.At least we got to see the new born baby!OMG.It was soo pink in colour and we saw how the nurse cut his umbillical cord(its a boy,woo hoo!)HAHA.The,she weighed him and took his temperature. He seemed healthy. And he was shivering btw,the place was air conditioned and was freaking cold!Poor guy.

And at last, after a partial good scolding from the boss,A.K.A DR SAID..We were freaked out man!He was lecturing us for loitering around, nevermind that story.
We had a briefing on what to do 'ON CALL'.They further divided us into 3 groups and I formed a group of my own,and i'm the ONLY BOY.HAHA.It was fun!Made new friends and had a good chat with May from Perak. She's(May) interesting to talk too and her motto is LIFE SUCKS.LOL.At 8PM,we started our rounds. My team was stationed at the red zone for 45 minutes. It was cool to watch how an endoscope work, and had a nice chat with Dr Ismail.A jovial guy explaining a lot on how being a doctor involves no extra time, not to make money or own big cars, but it a dedicated service to help others.Real liked his talk,it was very inspirational. And yeah, the endoscope set has the same value as a NEW TOYOTA CAMRY.Now,thats cool!

Our next rounds at the yellow and green zone were super boring until 3 buzzing sounds were heard!(ONE TIME BUZZ MEANS GREEN ZONE,NEXT IS YELLOW AND 3RD TIME IS RED ZONE)Almost everyone rushed to the red zone and a man was suspected to have a heart attack. The doctors immediately tried to stabilize him.The best part is another 3 buzzing sounds were heard and another patient came in!HE HAD A BLADE STUCK TO HIS ABDOMEN.OUCH!They were like covering the curtains and all, sadly we could not see what was happening.Whew, luckily,he was safe.He did not lose much blood. And then, when everything was back to normal,we headed to meet the Deputy Pengarah again for some advice. He explained that doctors don't have fixed lunch breaks and stuffs like that.Everday is a new day, hectic and THE PATIENT IS YOUR TEXTBOOK!At about 9.45pm,we were allowed to go back home!Wow,exhausting but superb 1st day.I will never forget this day man.Me and Charwinth stopped by a mamak restaurant for dinner before heading home. And guess what, I was totally tired and slept off within a few minutes after a good bath.Tring but fantastic day. Can't wait for day 2.
